Birth and Death Certificates may be obtained through our walk-in service, by mail or through www.vitalchek.com. For your convenience, you can process online requests through an independent company that MCHD has partnered with to provide you this service; VitalChek Network, Inc. Walk-In Service

We are open 47 hours a week; 8 a.m. - 5 p.m. Monday through Friday, and 9 a.m. - 7 p.m. Wednesdays from November 1st - February 28th and 9 a.m. - 8 p.m. March 1st - October 31st. Extended hours on Wednesdays allow the public better access to our services, including individual assistance to persons doing genealogy research. You MUST have valid identification to obtain Birth Certificate and payment is by cash, check, or money order (debit or credit cards are not accepted for in person purchases).

The Birth Certificate Request Form is for mail service orders only. The Birth or Death Certificate Request Form is not needed to obtain birth or death certificate in person at the office.
